Withdrawal of participation

I hereby wish to WITHDRAW my consent to participate in the Healthy Mum, Healthy Bub trial, and understand that such withdrawal WILL NOT jeopardise any current/future treatment or relationship with the University of Sydney, the Matilda Centre, the funders, or any health care provider.

I give permission for the Healthy Mum, Healthy Bub project team to retain any data collection as part of my participation to date, and to include it in analyses conducted as part of the overall study. If you do not give permission for this and want your previous data to be destroyed, please email the Healthy Mum, Healthy Bub team on healthy-mum-healthy-bub@sydney.edu.au
